The office is open from 8a.m.-5p.m., Monday-Friday (excluding university holidays). Be sure to give yourself ample time to obtain the OneCard as there will be other students getting their card. For location and details, please visit this link: https://admin.lmu.edu/onecard/ Ensure that you have either of the following ID’s available: -U.S. driver's license issued by DMV -State issued ID -Passport -U.S. Military card |

Students who wish to request a vaccination exemption for medical or religious reasons must complete the appropriate exemption form available through Vax LMU. Keep in mind, you’ll need to confirm that LMU received your vaccination paperwork and for those submitting an exemption - you will need to get tested twice weekly through LMU.
